Cancellation Policy:
1)Written Notice on the Membership Termination Form MUST BE submitted by the 28th of the month you plan to attend. If the membership termination form IS NOT completed by the 28th the form is not submitted your account will continue to be billed.
2)In the event you complete a Membership Termination Form, but you have an outstanding balance, your account will not be shut down until your balance is paid in full.
Membership Terms:
Fees continue to be withdrawn from checking account until the termination of membership paperwork is signed and submitted to the owners.
A $5 Service Fee will be enforced for all declined charges.
A one-time $20 fee will be charged for digital key fob setup.
It is the responsibility of the member to inform Iron City Barbell of new Payment Changes.
Corporate/employee add-on members must be immediate family members of the primary account holder.
A Maintenance fee will be taken out every March 1st and July 1st in the amount of:
- Single Membership - $20
- 2-Person Membership - $35
- 3-Person Membership - $45
- 4-Person Membership - $55
I hereby declare that I fully understand the terms set forth in this agreement. I fully understand that this is a set list and not an all-inclusive list of rules that I must abide by, I fully understand that a violation of any of the set rules or any other rules considered to be wrongful/inappropriate acts by Iron City Barbell may result in termination of my gym membership without refund.
- You and anyone entering the facility with you, are to swipe the access card to enter the facility. If you bring someone with you who is not a member, THEY MUST pay for a day pass, if they do
not, then your account will be charged for the day pass along with a .50 terminal fee. - I am not a Registered Sex Offender or been convicted of a violent crime.
- Any type of harassment of members will be grounds of immediate membership termination.
- No Soliciting, No Profanity, No Tobacco Use.
- Iron City Barbell IS NOT responsible for lost or stolen items.
- Customers when finished with equipment will clean equipment and place weights back on the racks from which they came.
- Shorts, Sweats and Shirts must always remain on. Work boots ARE NOT to be worn in the facility. If you wear muddy shoes, boots or any type of footwear they must be removed at the
door. - Children under 16 years of age are recommended by Iron City Barbell to come with a parent. Parents who sign this agreement are stating that they understand that Iron City Barbell WILL
NOT BE held responsible for accidents/injuries involving their children if they enter the facility by themselves/with friends and/or without a parent.
Release of Liability: Iron City Barbell, 232 East Huron Street, Jackson, Ohio 45640
The undersigned being of lawful age, As Client/Customer of Iron City Barbell states that they are an adult and understands the proper use of the facilities exercise equipment. The Client/Customer understands that exercising alone IS NOT recommended by Iron City Barbell and by signing this form releases Iron City Barbell of all liability.
The Customer/Client by signing this release of liability agrees to be responsible for damages caused by improper use of Iron City Barbells facility and/or Equipment. (Example: Slamming weights, dropping dumbbells.)
Client/Customer by signing this release of liability understands that the use of Iron City Barbell will be at their own risk, also Client/Customer understands that they should consult a physician prior to beginning any exercise program at Iron City Barbell and Iron City Barbell makes NO evaluation as to the client/customer’s health and fitness to engage in these programs.
Client/Customer agrees that the terms of this membership agreement/release of liability apply equally to all family members, individuals on membership plan to include minors, who use the facility and equipment as a guest or as part of the family membership.
If all of this is understood by all parties, that the changes of promises incorporated herein sufficient consideration to make this agreement and release binding all parties that this agreement and release constitutes the entire agreement regarding the use of Iron City Barbell’s exercise equipment and facility, and that this agreement and release is to be constructed under the laws of the state of Ohio.
